GUIDE Training Overview
Care Navigator Training
Nevvon provides specialized training for care navigators, who play a crucial role in the CMS GUIDE Model. These navigators are trained to support families by guiding them through the healthcare system, coordinating services, and ensuring that care plans are followed effectively.
Family Caregiver Training
Nevvon's family caregiver training covers essential areas such as emergency services, home safety, assistance with ADLs, and managing the behavioral and psychosocial symptoms of dementia. Caregivers are also trained to navigate the care continuum, work with healthcare providers, plan for the future, and manage their own stress and well-being.
Dementia Training
Our dementia training is extensive and includes content approved by the Alzheimer's Association. Caregivers receive comprehensive information on dementia diagnosis, common behavioral changes, and available resources. All materials are designed to be easily understood and can be provided in the primary language of the GUIDE Beneficiary and Caregiver.
